This manual contains important information for persons in FNS headquarters, FNS regional offices, and distributing agencies, which include state distributing agencies and Indian Tribal Organizations that are charged with the responsibility of providing USDA Foods to disaster relief organizations in the event of a disaster, emergency, or situation of distress.

Report of the Child and Adult Care Food Program for state agency submission to FNS

The ITO/state agency must submit Form FNS-152, Monthly Distribution of Donated Foods to Family Units,  that documents the data obtained in the physical inventory with the perpetual inventory record. Shortages must be reported to the appropriate FNS regional office.

Form FNS-674 is used to request access to the USDA Food Program Reporting System (FPRS).  

FNS Form 292A is to be used to report Commodity Distribution for Disaster Relief. 

Form to provide data necessary to monitor the state revenue matching requirement under the National School Lunch Program.

It does not replace or supersede federal regulations or policies that govern the WIC Program.
