Instructions for Child and Adult Care Food Program Centers, Sponsoring Organizations and Family Day Care Home Providers

FNS Form 292A is to be used to report Commodity Distribution for Disaster Relief. 

School food authorities (SFAs) participating in the National School Lunch and School Breakfast programs are required to verify income for a small percentage of households approved for free or reduced price meals each school year. However, getting households to respond to verification requests can be challenging for SFA staff. If households do not respond, they lose their benefits regardless of their actual eligibility.

State agencies apply for reimbursement using Form FSA-21, submitted to the appropriate FNS regional office for approval. 

This certification is required by the regulations implementing Executive Order 12549, Debarment and Suspension, 7 CFR part 3017, Section 3017.510, Participants' responsibilities. The regulations were published as Part IV of the Jan. 30, 1989, Federal Register (pages 4722-4733).
