This agreement sets out the requirements for administering the child nutrition programs and the food distribution programs.

Form FNS-7 "Destination Data for Delivery of Donated Foods" is to be used for reporting in conjunction with FNS Instruction 709-5.  

A compilation of resources for operators of USDA Foods in Schools and child nutrition programs.  

Form FNS-674 is used to request access to the USDA Food Program Reporting System (FPRS).  

FNS Form 292A is to be used to report Commodity Distribution for Disaster Relief. 

The list contains the contact information for each state administering the unprocessed fruit and vegetable pilot, as well as the main USDA contacts.

Form FNS-57 is used by consignees to report overages and shortages in a shipment and commodities damaged in transit.

State agencies apply for reimbursement using Form FSA-21, submitted to the appropriate FNS regional office for approval.
