This memorandum provides additional information regarding the provisions related to the frequency and number of reviews for state agencies monitoring the Child and Adult Care Food Program, that were codified in the Child Nutrition Program Integrity Final Rule published on Aug.23, 2023. These provisions have a compliance date of Aug. 23, 2024.

This memo provides reporting guidance for the FNS-640 report beginning with reporting for school year (SY) 2018-19 due March 1, 2020.

The purpose of this memorandum is to provide additional information on the 5-year review cycle provision of the Child Nutrition Program Integrity final rule. This memorandum relates to program-specific changes in the National School Lunch Program and School Breakfast Program.

Attached to this memorandum, please find a revised edition of the Prototype Application for Free and Reduced Price School Meals, with an accompanying instructions document. These materials may be adapted for direct use by state and local agencies, or as a reference for designing an effective application packet that meets all statutory and regulatory requirements. 

This information collection is mandated by section 231 of the Healthy, Hunger-Free Kids Act of 2010. Section 231 of the HHFKA requires USDA to implement a program to recognize exemplary breastfeeding support practices at WIC local agencies and clinics.

The Healthy, Hunger-Free Kids Act of 2010 (the Act), PL 111-296, established a new reporting requirement for the WIC Program. USDA is now mandated to compile and publish annually, breastfeeding performance measurements based on program participant data on the number of partially and fully breastfed infants for each WIC state and local agency.

This notice invites the general public and other public agencies to comment on this proposed information collection for activities associated with administering emergency allotments (EA) waivers issued by USDA based on a public health emergency declaration by the Secretary of Health and Human Services related to an outbreak of COVID-19.
