Attached to this memorandum, please find a revised edition of the Prototype Application for Free and Reduced Price School Meals, with an accompanying instructions document. These materials may be adapted for direct use by state and local agencies, or as a reference for designing an effective application packet that meets all statutory and regulatory requirements. 

This fact sheet, available in both English and Spanish, reviews the steps to successfully integrate local food in child nutrition programs.

The previous version of this memorandum contained incorrect references to state agencies throughout. FNS is correcting this memorandum to remove the incorrect references and so that it more clearly and accurately reflects the federal regulations. FNS issued eight child nutrition programs off-site monitoring fact sheets to assist states and sponsors in conducting off-site monitoring of child nutrition programs during the pandemic. These documents include a fact sheet that has background information on all programs, as well as separate facts sheets for state and local operators for each program.
