Attached to this memorandum, please find a revised edition of the Prototype Application for Free and Reduced Price School Meals, with an accompanying instructions document. These materials may be adapted for direct use by state and local agencies, or as a reference for designing an effective application packet that meets all statutory and regulatory requirements.

School programs policy memo SP 14-2019 provides SY 2017-18 reporting guidance for the FNS-640 report. The FNS-640 form became available in FPRS on March 1, 2019. In order to update and streamline policy guidance for the USDA Foods Processing Program, FNS is cancelling policy memoranda FD-009, FD-025, FD-130, FD-102, FD-030, FD-038, FD-048, FD-103, and FD-108. The guidance provided by these memoranda is outdated and has been superseded by the Final Rule: Revisions and Clarifications in Requirements for the Processing of Donated Foods, which was published on May 1, 2018.

This school year updates to the AR process were only made in areas regarding the Fiscal Action process and Resource Management.
