Food Stamp Retailer Application Request
Local USDA, Food and Nutrition Service (FNS) field offices are responsible for the authorization and monitoring of retail firms participating in the Food Stamp Program. FNS field offices are located throughout the United States and cover stores located in the continental United States, the District of Columbia, Guam and the U.S. Virgin Islands.
To download a free application package just click on the USA
State Map and select the State or US Territory where the retail store is located or select from the State or US Territory list.
Because some States have multiple FNS field offices, you may be required to select the county where your store is located. The application package documents you need to download consist of three items: (1) the application form; (2) Store Eligibility Criteria and Application Checklist, and (3) a letter from the FNS field office that will process your application.
