Prescreening Tool for Medical Expenses
Vermont

The Central Vermont Council on Aging developed a prescreening tool that helps to maximize documentation of medical expenses. This tool was developed in the fall of 2004 and is used mainly in Washington and Orange counties. The medical information is kept in the client’s file and can be used as a reminder for clients that have problems remembering their medical expenses. The main problem with this form is that the print is very small and clients may have problems reading it by themselves. See attached form.

Suggestion: Enlarge the print on the form or help the client.
